Restaurant Inspections

Information is from state Health Department records. All reports are from regular food service inspections unless otherwise noted. Critical violations are defined as items relating directly to factors that lead to food-borne illness and must be corrected immediately. Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.

Rainbow House Asian Bistro

Follow-up inspection

Critical violations: Active managerial controls to maintain food safety were not observed during time of inspection. There was a child in the kitchen. Raw pork was being stored next to cooked chicken on the prep table. Breading is being stored on cardboard lids from cardboard crates. Rice at 108 degrees, chicken at 80 degrees in walk-in cooler, prepared at noon; cool food in small batches or thin pans to promote rapid cooling; sauce with vegetables at 99 degrees prepared at noon. Chicken at 50 and 54 degrees, egg roll at 54 degrees in refrigerator facing the kitchen.

Noncritical violations: Wiping cloths shall be used as intended and in compliance with regulations pertaining to retail food establishment; wiping cloths must be stored in a bucket of sanitizer.

La Hacienda

2010 E. Main St., Siloam Springs

Critical violations: Employee did not wash hands between dirty and clean dishes. Employee handled ready-to-eat food with bare hands; reminded employee that gloves or utensils must be used when food is not going to be cooked again. Carton of raw eggs were being stored above guacamole in the upright refrigerator. Food items checked were not holding at 41 degrees or below in the prep table; check unit to ensure it is working properly and adjust as needed. Items not date marked as needed; date marking for ready-to-eat, potentially hazardous and time/temperature control for safety food prepared on site or opened commercial containers held for more than 24 hours.

Noncritical violations: Pans of chicken and beef are not covered in the walk-in; shelves above the items have some food on them and can fall into uncovered food below (repeat violation). Pan of beef stored inside of another uncovered pan of beef; bottom of top pan was touching the food in the other pan. Food buildup in the bottom of the reach-in cooler, especially in the back of the unit. Bottom inside of both upright freezers are not clean; food or other items spilled on the bottom (repeat violation); must be defrosted and cleaned properly. Standing water in the bottom of the reach-in prep table; it could be from melting ice, or an issue with the unit draining properly.

McDonald's

203 N. Curtis Ave., Pea Ridge

Critical violations: Require food employees to wash their hands using the procedure delineated in the Arkansas Rules and Regulations; employee did not wash hands prior to gloving. No sanitizer for wipe buckets upon arrival for inspection. Wiping cloths not stored in sanitizer, only soap.

Noncritical violations: Frozen food and food wrappers were on the floor in the walk-in freezer.
