PEA RIDGE — The seasonal flu clinic for Pea Ridge District students will be on Wednesday, Oct. 12, in the Pea Ridge Primary School Gym. This is voluntary, so for any students that wish to participate, parents must return the vaccination forms to school as soon as possible.
You should be receiving these forms in your student’s backpacks. If you do not see one by Friday, call your campus nurse for a second set. We would like to have the forms returned by Oct. 7 if at all possible.
The students will be bused to the Primary and back from the other campuses with chaperones.
The Benton County Health Department supplies the flu vaccine and will be assisting with the clinic. Mist and injectable will be available for the students but the quantity of each type is not known until the Health Department gets supplied. The mist will be given out first from youngest to oldest groups of students.
There is an area on the form to check which type you wish your student to receive. We will not give what is not checked. Make sure signatures are on the forms or the vaccine will not be given.

THE FORMS MUST BE COMPLETE.
If you have any concerns or questions, do not hesitate to call a campus nurse or speak with yourstudent’s personal physician. This is a one day clinic. If your child is ill, they can go to the mass clinic given by the Health Department. A note with the vaccine date will be sent home with your student on clinic day. Only students enrolled in the district are eligible for vaccine.
Younger siblings not in school yet may not participate. Staff may also receive the vaccine. Parents arebeen welcome to be with their student on clinic day, but we have found they do very well with the Nursing staff attending them and the clinic flows smoothly as well.
